Comment remplir l'attestation d'assujetissement de la mutuelle ?

Les mutuelles demandent à l'employeur de remplir une "attestation d'assujettissement" pour les nouveaux professeurs temporaires. - I know what this is, it's a document proving that the person is subject to social security tax (in Belgium). But how do you word this in English?
