English to Chinese (simplified and traditional); 42,000 words, insurance documents

We have a number of insurance policy documents written in English (total word count: 42,000 words), which are to be translated into Chinese (simplified and traditional). Since there is a significant amount of repeats in these documents, we are looking for translators and proofreaders who can use Trados.

If you are interested, please respond by providing us with the following material / information:
(1) Your CV
(2) Your location (city / country)
(3) Your language variant (simplified or traditional)
(4) Version name/year of your Trados
(5) Your translation rates (for unique segments, 100% repeats and fuzzy segments)
(6) Your proofreading rate
(7) If we assign the primary translation work to you on January 5, 2018, how many words can you translate by January 19, 2018?
(8) Whether or not you accept payments through PayPal
