Although total hip arthroplasty continues to be a very successful procedure to alleviate pain and restore function in patients with arthritis of the hip, osteolysis of pelvic bone stock is a common sequel to acetabular component loosening in total hip arthroplasty. The acetabular defects are often very significant and present a problem to the reconstructive surgeon undertaking revisión arthroplasty.
The principles of acetabular revision include stable bone coverage that can support the new acetabular component, restoration of the anatomy and bone stock for future revisions, and equalizing leg length. The approach to achieving these goals depends on the severity of the pelvic defects.
Several classification schemes for describing acetabular defects have been described. Perhaps, the most widely referred to is the American Academy of Orthopaedics Surgeon ( AAOS) classification: Type I ( segmental) defects and Type II ( cavitary) defects can be successfully reconstructed using bone-graft and cemented or cementless components. Use of structural graft has been recommended for reconsruction of Type III ( combined) defects, In cases of pelvic discontinuity or when the bone loss severity and geometry do not favour an uncemented porous socket, antiprotusio cages with structural bone graft are considered. Cages provide a large surface against the pelvis to span bone defects, distributed load, protect large bone grafts, and resist early migration.

The purpose of this chapter is to provide information about the current indications for antiprotusio cages, to explain the technical features, and to report our results.
