We report the clinical case of a 62 year-old man presenting with scalp multiple skin lesions developing on the preauricular and high cervical areas for around 40 years. These lesions correspond to pink, hard and non-painful nodules at palpation. Their large number causes capillary health disorders and discomfort when wearing a motorcycle helmet.
Patient's history shows that several surgical exeresis were performed by various teams for more than 20 years. He presents therefore with many scarred scalp areas. An anatomical pathology was carried out on one of these exeresis and evidenced a lesion presenting basophils with a disorganized aspect and topped by eosinophils with positive PAS. It equates cylindroma-specific histological aspect (3-4). Lesions large number and location made us diagnose a scalp cylindromatosis or turban tumor syndrome.
Regarding the risk of malignant degeneration, patient physical discomfort and demand for a one-stage radical treatment, we chose to perform a total scalp excision followed by a split thickness skin graft.
