This is a section on a medical report from a medical clinic in Canada. I'm likely to translate it literally as "plan notes", though not convinced that is correct (based on Google searches). Can't find an actual equivalent in any online tools or searches. Just figured I'd ask. Thanks.

If this refers to medicine, in general. If not, it might be something like "therapy progress notes", like in the case of psychological treatment.
