My comment:

surnames usually remain unchanged. However, the meaning in this case is commected to the italian spedale/spitale, an ancient form for "hospital". The ancient term was also used to mean "orphanage"

Thank you. Might the presence in ``Spedaliere' of the ``-iere' suffix indicate -- same as in ``consigliere' -- a trade or profession? Might ``spedaliere' be loosely interpreted to mean caregiver perhaps?
