My comment:

Secondo me le immagini sono valutate dal medico che sta sul posto (on site) ed esegue l'esame e da un altro medico, che, invece, legge il referto solo in un secondo momento (per questo 'off site'). Trova una soluzione, perché una traduzione con un solo termine non è possibile. Non userei nemmeno 'lettore' per reader.
