casco de una ciudad is its central area or the heart (a nice, affectionate expression used in many touristic guides). Other instances of the use of casco: for example casco urbano would be the urban or build-up area

Casco in Spanish has meanings, a helmet or a horse 's hoof, but in reference to urban issues it mean the part of town, in this case it's the historic antique part of town.
