My comment:

Leave it to the French, they will codify everything! “Tiers temps” is one of these terms which have become fashionable lately and is being used for multiple meanings. Here they are:
1- A hockey period, since each game is divided in 3 periods.
2- A part-time activity that takes 1/3 of full time.
3- An additional period given handicapped students to finish their exams
4- The time devoted to activities which are not considered “work” (i.e. paid for by an employer) nor “leisure or rest or recreation”, which we would call “civic activities”
5- The time given (and paid) to professionals (teachers, doctors etc.) to perform extracurricular activities (e.g. the Math teacher who is also the basketball coach). This is the meaning in this case, and the translation is not easy. But here is what I’d say:
In the time allotted to the doctor for non-clinical activities, he is implementing an anti-smoking campaign
