This collocation appears at the end of a Cardiology discharge letter. Any ideas?

My comment:

The IATE shows "discharge" and "opening". If he or she had a Cardiac Cath to open an artery it could be that it was not 100% successful.... Just an idea

This fits the bill nicely, Isabel, as the patient did indeen go to the cath lab and the operator was unable to clear the critical lesion in the LAD.
