Keywords:

..."mal-a-dit" pour comprendre que la maladie vous dit quelque chose...This is in an essay about how psychological confliect can cause one to be susceptible to cancer...The best I can come up with is "warning call" since it seems that the idea is to get that kind of conflict out of your life because of its negative impact, and here the author seems to be saying to not look at it in a totally negative light, i.e., to see some value in it.

That's the idea, "the pain is telling" you something. Play on words are notoriously impossible to translate, your interpretation is as good as any.

I may never know what it is they really wanted, but based on the rext of the context, I'd say your and my interpretation is apt enough, anyway.
