My comment:

it means " failure of an item caused either directly or indirectly by the failure of another item " - can be in medicine, electronics, automatics etc.

'fallo' can mean 'failure' as in 'fallo tcnico' or 'decision' as in 'fallo legal' the context is insufficient to work out which one it is. it appears that the first drug didn't work (ie. failed) and a decision has been taken to prescribe a second drug possibly by a second doctor, in which case it would be a second opinion. however, more context is definitely needed
