When the term is in the death certificate, you must use the the "legal" term: Affiant. You are giving a declaration (declaracion) and cannot be only a simple comunication. The person must "declare", be the witness that such person is dead. The death certificate is an official document.

An affiant is somebody whose signature appears on an affidavit. I am a Legal translator, not a wannabe.
