My comment:

La expresion originariamente se referia a los locos en el manicomio. No todos los locos estan en el manicomio y no todos los que se encuentran en el manicomio estan locos. Es una expresion que indica que en una clasificacion siempre habra elementos que no deberian estar y otros que deberian estar, se encuentran excluidos de la misma.

My comment:

I think there might be a typo. This is an old proverb and it goes "No son todas las que están ni están todas las que son." Basically the meaning is that looks might be deceiving, but you cannot really translate it literally.
