Let’s use some logic here. “Momento” refers to a point in time, “inicial” means initial, “referencia” means reference. So the literal translation should be Initial point or reference point. Point in English is probably not sufficiently explanatory, so some further descriptive term would be used, I’d use Initial Assessment or Reference Assessment as the title for the page. For “Visita inicial de referencia”, I would say Reference first visit.

My comment:

I am not exactly sure on this, but am fairly sure that it is not "initial moment" when dealing with a doctor's visit or appointment. I think "referencia" would refer to a doctor's referal for a medicine or an appointment.
