So you can understand how the Holter is used (I have worn one myself), it is a heart monitor that is worn hanging from from a belt around one's waist. There are wires that go from the monitor to electrodes that are attached to the patient's chest. If the patient is male, then his chest would have to be shaved before the electrodes can be attached. The Holter is usually worn for several days so that the hear'ts rhythms can be monitored during sleep and different kinds of activities. The diary is used for the patient to write down when s/he feels the heart beating irregularly, or when s/he is performing some kind of strenuous activity, or just resting or sleeping.
