Chronic Condition Records.

My comment:

Medical jargon would have "Chronic Condition". As opposed to "Acute Condition" for sometning like appendicitis...

ALD patient record card (long-term disease) / ALD patient index card

My comment:

Affection Longue Durée (ALD) -> bénéficier d'une prise en charge à 100 % des soins

In case of a disease requiring prolonged and expensive treatment a 100% coverage of the treatment costs may be possible! In order to benefit the attending physician prepares a treatment plan specifying the required therapies and also applies for reimbursement of the costs (i.e. the costs of medical care and treatment related to this long-term disease). To do this, he fills out a form called a "medical treatment protocol" ("protocole de soins").
