My comment:

I get the impression the examiner is with a patient in distress (I note the word, "larmes"). So the auscultation of the chest would be difficult (between sobs). Likewise the examination of the abdomen would not be easy with someone who is not relaxed. That's my take on it anyway.

I think the abdomen releases a clear liquid which resembles tears which makes the area supple and easier to cut. I hope this is your answer.
