The literal meaning is "very much in a spirit of brotherhood", and it is the way a French doctor signs off a letter to a COLLEAGUE, i.e. to another doctor. This is the standard format when closing a hospital discharge letter to a family physician reporting on a patient's clinical course while an inpatient.
