Context:

Noting the Duke´s discomfiture, the doctor stepped forward with a sideways glance to check that no one was within hearing distance, and cleared his throat nervously.

Literary piece of writing. Again, I am desperately looking for a NATURAL way of conveying this in Spanish. I even figure out the movements in the doctor´s mouth. However, I need the exact words in Spanish for this. A literal translation wouldn´t work, as usual.

My comment:

Me resulta intrincado traducir esta expresión en pocas palabras. Es cuando una persona tose una o dos veces breves para salir de una situación incómoda. Por eso sugiero "nerviosamente", podría ser "tosió incómodo" y es posible que se puedan encontrar sugerencias más apropiadas. ¡Suerte!

también se dice "aclararse la garganta" o incluso "carraspear para aclararse la garganta (o: aclarar la voz)"
