I understand it as an expression of relief. He smokes thinking that he came to a close call on whatever the situation was.

Does "la" refer to a person (su mujer) or to an occasion, or whatever else?

Unfortunately, I'm not very clear about what the "la" refers to, either, but I was thinking that it has to do with getting rid of his wife, with whom he has had a dull relationship for years, because, at this point, he's actually starting a new relationship (just kissing, at this point) with someone else, on the sly. Other than that, perhaps it's the new experience that he thinks is liberating. Not really sure, as you can tell. Because it will become clearer as I go along.
