Augustina le hizo señas para que entrase en su casa sin que es enteraran los vecinos. La cogió de la mano, como si fuera una niña. Luego, una vez cerrada la puerta, miró por el pestillo por si había alguien fuera. Isolina se asustó. Nunca la había visto así. Entonces entendió que Aníbal no había fallecido, porque estaba mirando que no llegase de repente.

My comment:

He's trying to see who's outside, so it would make more sense to me that he would be looking through the hole rather than "at the bolt"

It then moves to say that Isolina (female name) had never seen her like that. Then she -Isolina- understood that Anibal had not died, and she -Augustina- was making sure he didn´t arrive unexpectedly.
