"fakes and pains" is a humorous rewording of the expression "aches and pains"--which is used to describe ordinary, garden-variety pain, especially when referring to someone who complains about pain a lot. Depending on the context, "fakes" could be fake or pretended pain.

Se trata de un juego de palabras con la frase hecha "aches and pains" dolores y sufrimientos. Están jugando con el mismo sonido para referirse posiblemente a algo falso o a algún impostor, debido a que se trata de tira cómica.

My comment:

Creo que no es una frase hecha. Al tratarse de comics, podria ser que fake se refiriera en vez del adjetivo falso, a una persona: farsante, embustera, impostora
Pain se refiere a penas o sufrimientos.

Para dar una respuesta lo más certera posible habría que leer el artículo para conocer su contenido.

Sin embargo, podemos decir que fakes o fake significa falsedad y pains es el plural para pain que significa dolor.

Un texto con la frase "fake and pain" es el siguiente: "get in front of someone and cause them to rear end you and then sue out of the for damages to the vehicle, injuries both real and fake and pain and suffering and fictitious lost wages."

También podría ser: "mentiras y dolores"
