as amended

My comment:

I have had to translate this precise language within the context of doing legal work in law firms in London.

The most accurate way of referring to this is by using "as amended", preferably immediately after the Articles of the Family law you will indicate.

My references:

Stephenson Harwood, two partners, one of them having "read" law in Cambridge.

at the dates or as of the dates

My comment:

Whereas the Family Code as amended and completed at the dates given in its articles.
À ce jour: to date, à ces jours, at the dates [etc]

There is more than one date. Various articles were amended at or on different DATES, plural.
